ABOUT CHRONICLES

EXPANDING ACCESS. ONE COMMUNITY AT A TIME.

CHroniCles, a special project of the RCHN Community Health Foundation, the Geiger Gibson Program in Community Health Policy at the Milken Institute School of Public Health and Health Services at The George Washington University, and the National Association of Community Health Centers, is dedicated to highlighting and recording the contributions made by community health centers across the country in delivering high-quality and comprehensive primary health care services to 23 million people each year.

The goal of CHroniCles is to record the vibrant, diverse, and important living history of each community health center and of the health center movement as a whole, and to make accessible to visitors everywhere. Every health center story is unique, and reflects the needs and aspirations of its community. CHroniCles aims to honor the rich heritage and ongoing work of community health centers by having each health center document its own story to create an on-line, web-based tapestry.

The site showcases contributed narratives, photos, data and other materials to advance an understanding and awareness of the vital role that community health centers play in the American health care system. CHroniCles celebrates the distinctive history of the health center movement, and the significant role community health centers fulfill in the lives of millions across the country.

HIGH-QUALITY HEALTHCARE

Community Health Centers provide effective, comprehensive person-centered preventive and primary care services and serve as medical homes for their patients.

COMMUNITIES FIRST

Under the leadership of community-based boards, a majority of whose members receive care at the center, health centers tailor their programs and services to meet the unique clinical and cultural needs of those they serve.

SUSTAINABLE IMPACT

Health centers lower total health care costs and generate economic opportunities in the communities they serve by providing good jobs for local residents, creating an important economic engine.

BEYOND MEDICINE

In addition to providing excellent clinical care, health centers support healthy communities by ensuring access, building and fostering trusting relationships with their patients, and weaving a strong thread through the fabric of the community.
